Green Buddha Patient Network Application
In order to become a member of the Green Buddha Patient Co-op several bits of (digital) paper are required: 1) Current contact information or a completed Green Buddha Patient Network Application, 2) a copy of your medical cannabis authorization and 3) a copy of your photo ID. Once you have these three documents you maybe fax or email them into our office to initiate the processing of becoming a member.
Washington State Medical Association Medical Marijuana Authorization
We encourage you to use the Washington State Medical Association form for your authorization which is available here. We also ask that you write your current telephone number on the copy of your authorization you send in.
Photocopy of a Washington State photo ID
This can include a Washington State Driver's license or Washington State ID. Other forms of ID might also be allowed, as long as verification that one is a Washington State resident. You will need to bring this photo ID with you every time you visit Green Buddha Patient Network.
